Background: Polycystic ovarian syndrome (PCOS), or ovarian hyperandrogenism, has been identified in increasing numbers of adolescent girls. PCOS is manifested by evidence of ovarian androgen excess (hirsuitism, acne), chronic anovulation, and ultrasonographic evidence of ovarian cysts. It may be associated with obesity and insulin resistance. We sought to characterize the presentations of PCOS in the patients seen in our multidisciplinary adolescent PCOS clinic in its first 33 months of existence. Polycystic ovary syndrome (PCOS) is a common endocrine disorder affecting 5–10% of adolescent girls. Girls with PCOS typically present with irregular menses, acne, hirsutism, and obesity. The stigmata of PCOS often are distressing for the adolescent girl and may negatively impact her quality of life. Good communication with patients and their parents is important, because girls' main concerns are usually related to irregular menstrual cycles and excess body hair, whereas treating clinicians are concerned with symptom management and prevention of long-term sequelae. Hormonal contraception, through suppression of ovarian activity or endometrial proliferation, can be used to treat and prevent a range of gynaecological disorders. There is evidence to support the effectiveness of the levonorgestrel intra-uterine system (LNG-IUS), combined oral contraceptive pill (COC) and progestogen-only injectable depot-medroxyprogesterone acetate for treating heavy menstrual bleeding and dysmenorrhoea. COCs are also an effective treatment for acne and hirsuitism, and may lessen the symptoms of premenstrual syndrome. There is good evidence that COCs can significantly reduce the risk of both ovarian and endometrial cancer.
